Why does the blended rate matter?

Because it shows whether work sat at the right level. A matter billed largely at partner rates when much of it was paralegal work is expensive for the client and inefficient for the firm.

Can I mark up disbursements?

In most jurisdictions no, or only with disclosure and consent. They are the client's costs advanced on their behalf, and the rules are specific. Check yours.
