Does this include profit?

Only the income you named. If you want the business to earn something beyond your own wage, add that to the income figure or treat it as a separate line.

Should I actually quote by the hour?

Often not. Customers compare hourly rates and cannot judge them. This rate is how you check that a flat price is high enough, not necessarily what you put on the quote.
